NHS Social Prescribing Link Workers and Hounslow Council Falls Prevention are collaborating to improve residents’ well-being through group meet-ups.
When: Every Tuesday from 4.30pm to 6pm (4 sessions), starting Tuesday 10 October 2023
Location: Feltham Centre for Health, 210 High Street Feltham TW13 4GU
This event is aimed at residents registered with a GP in Feltham and Bedfont areas, aged 50+ and taking four or more medications.
However, all are welcome to attend.
There will be four weekly sessions led by an NHS Social Prescriber and Hounslow Council's Falls Prevention Manager, taking an individual approach to health and wellbeing.
The group sessions will focus on different topics each week including, falls prevention, diabetes, dementia, managing stress, isolation, social interaction, Lasting Power of Attorney (LPA) and wills. These sessions will be interactive with the opportunity for people to connect with others, learn about local support, and the tips and tricks they can use to stay healthy.
