In 2016 the prevalence of diabetes recorded by a GP in Hounslow was 7.3% of the adult population (17 and over), equating to17,519 residents. However, the prevalence estimate of diabetes in Hounslow is 9.6% which accounts for 23,125 people aged over, and this means there are over 5,600 people with diabetes who are not diagnosed but could be. In addition, diabetes prevalence in Hounslow is predicted to increase to 10% in 2020 and11% in 2030.

That's why we are raising awareness of the risk of developing type 2 diabetes and how to reduce it.

The Diabetes Service in Hounslow provides the following support:

Diabetes management and specialist support in GP practice

  • Telephone advice line and email advice service for clinicians
  • Structured education sessions – Nationally accredited X-Pert diabetes Programme for people with Type 2 diabetes and those with Type 2 diabetes using Insulin
  • Domiciliary visits for housebound clients

Patients with diabetes are treated by their GP with support from a Diabetes Specialist Nurse allocated to the practice. However, based at the Heart of Hounslow Centre for Health, the specialist team can provide specialist care for cases too complex to be dealt with by a patient's GP practise but not tough enough to warrant hospital admission.
