Hounslow launches Enhanced Dementia Care Service
Health and social care partners across Hounslow are using Dementia Action Week to launch a new service aimed at people living with dementia in our borough.
The Enhanced Dementia Care Service (EDCS) is a new initiative from members of the Hounslow Borough Based Partnership.
It will provide a range of community-based support, including helping residents with a diagnosis to help prevent crisis and emergency hospital admissions. The service will help more Hounslow residents who are living with dementia, to live independently.
The Borough Based Partnership has produced a special video to explain the EDCS.
