Take this short survey by London Borough of Hounslow to share your experience on how you access and use your pharmacy.

LBH wants to find out what's working well for you and what could be improved within the services provided to you by pharmacies. In order to assess the health needs in Hounslow, and further develop pharmacy services, your voice matters.

All your answers will be kept confidential, so don't hesitate to share your thoughts!
