NHS North West London Community Services Review

NHS North West London has launched a vital review of community services to better support the frail population, especially older residents.

With 10-15% of those aged 65 years or more, experiencing moderate to severe frailty, it's crucial we ensure comprehensive, proactive care for the affected individuals.

Let's shape a high-quality, equitable frailty service accessible to all residents aged over 18 and registered with a NWL GP practice.
