The purpose of this evaluative survey is to review the impact of the Personal Care Framework (PCF) being embedded in the service and care provision among providers and how this has been received by care users and their representatives.

This review will examines the progress that local Practices in Hounslow have been making towards achieving the Hounslow Clinical Commissioning Group’s aim to introduce proactive Care Plans for some of the more Vulnerable Adults in each of the Practices with GPs as pivotal points in the care and management of the health of these patients to help them stay out of hospital, maximise patient involvement and reduce unnecessary admissions.

An evaluative review of the community recovery service and Integrated Community Response Service in Hounslow by Healthwatch Hounslow in 2016.

Ashgrove Care Home (ACH) has accommodation for 50 residents at present and they have 49 residents at the moment.

IN 2016, Hounslow CCG and the London Borough of Hounslow asked Healthwatch Hounslow to undertake a report on the Personal Care Framework (PCF) with the original objective being to evaluate how successfully the PCF has been implemented in the borough.

In early 2016 Healthwatch Hounslow ran an evaluative survey to examine mental health service provisions available and being accessed by children and young people (“C&YP”) in Hounslow.

In June 2016, Healthwatch Hounslow carried out a survey on primary care GP access for patients in Hounslow. This patient survey was commissioned by the Clinical Commissioning Group in Hounslow.

The Mystery Shoppers Pharmacy evaluation project was carried out in 2015 and 2016 to evaluate the working of the pharmacies in the Hounslow borough using Mystery Shopper scenarios.

The aim of this evaluation was to understand how Greenrod Place and Park Lodge House function as Extra Care services, whilst catering to the needs of their residents in Hounslow.
