Patient Experiences and Expectations of Blood Pressure Management
Imperial College Health Partners (ICHP) and North West London Integrated Care Board (NWL ICB) are leading a programme of work that aims to reduce cardiovascular disease incidents in North West London by improving blood pressure management.
They want to hear patient experiences and expectations of blood pressure management. This will help them to develop solutions that improve confidence in self-managing blood pressure
The survey is open for anyone who has been diagnosed with high blood pressure (hypertension), high cholesterol, experienced a stroke or cardiovascular event, and who lives in, or receives care in North West London.
