Falls Prevention - New Project
Hounslow Public Health, and other key stakeholders would like to understand more around awareness of the service and barriers to self-referral, and for those who have used the service, any issues around timescales for referrals and how the referral pathway operates.
The Hounslow Falls Prevention and Bone Health Service is dedicated to minimising the risk of falls and fragility fractures among adults aged 65 and above in Hounslow.
Targeting those at risk or fearful of falling, as well as individuals aged 50 and above for bone health management, the service adopts a preventative approach.
Collaborating with the Frailty Team at West Middlesex Hospital and the Home Safety Team in the London Borough of Hounslow, the program aims to enhance understanding of fall risks and empower individuals to reduce them.
By addressing falls and bone health comprehensively, the service ultimately seeks to prevent future falls, reduce hospital attendance and admission, lower the incidence of hip fractures, and promote long-term well-being and independence.
